ABSTRACT

Since the first edition of Tw-DRGs was developed in Taiwan in 2002, the system introduction of the hospital management level has been successfully completed. This paper detailed the organizational structure of DRGs case management in local hospitals ( DRGs promotion group, front-end medical care, ICD end, and the medical expenses reporting). It also introduced the operation process of identifying, locking and adjusting process of the pre-hospital, in-hospital and post-hospital aspects of inpatients, and provides in the end useful experiences for the promotion of DRGs case management in China′s mainland hospitals.

ABSTRACT

OBJECTIVE:To investigate the effects of MTP gene polymorphism on the lipid-regulating effect of simvastatin in the treatment of T2DM complicating with lipid metabolism disorder. METHODS:120 T2DM inpatients with hypercholesterolemia were selected from our hospital during Jun. to Dec. 2015 and given Metformin hydrochloride sustained-release tablets,Enalapril ma-leate tablets and Simvastatin tablets. PCR-RFLP was used to detect MTP G493T genotype. The levels of TG,TC,LDL-C and HDL-C were detected by AU 400 automatic biochemistry analyzer before treatment and the 4th week after treatment. The effects of different genotype on the change of blood lipid level was analyzed. RESULTS:Among 120 patients,the patients with MTP G493T GG,GT,TT genotypes accounted for 61.67%,26.67% and 11.67%,respectively,meeting Hardy-Weinberg balance(P>0.05). Before treatment,there was no statistical significance in TC,TG,LDL-C and HDL-C among different genotypes(P>0.05). After 4 weeks of treatment,the decrease of TC and LDL-C in TT genotype patients were lower than that in GG and GT genotype,with statistical significance(P>0.05). There was no significant difference in the decrease of TC and LDL-C between GT and GG geno-type,the decrease of TG and HDL-C(P>0.05). There was no significant difference in the incidence of ADR among different geno-types(P>0.05). CONCLUSIONS:After simvastatin treatment,the improvement of TC and LDL-C in patients with TT genotype is poor. MTP G493T gene polymorphism may be associated with the lipid-regulating effect of simvastatin in the treatment of T2DM patients with lipid metabolism disorder.

ABSTRACT

Objective To explore the role of the theory of human care in cancer patients during chemotherapy.Methods 150 patients with tumor undergoing chemotherapy were divided randomly into the intervention group and the control group (each had 75 cases).The intervention group took the human nursing care,the control group was applied with the traditional nursing method,compliance,liquid extravasation or phlebitis after chemotherapy were observed and compared.Results The compliance,incidence of liquid extravasation or phlebitis in the intervention group was significantly better than those of the control group.Conclusions Implementation of the nursing care based on human care theory can improve compliance of chemotherapy,satisfaction with the quality of care and quality of life in cancer patients,and hence is worthy of being recommended in clinical practice.

ABSTRACT

Objective To analyze the relationship between genetic variability and evolution among Trypanosoma brucei (including T. b. brucei, T. b. rhodesiense and T. b. gambiense), T. evansi and T. equiperdum isolates. Methods Genomic DNAs of 26 trypanosome isolates were amplified by a mobile genetic elements (MGE)-PCR technique and cluster analysis was performed based on the molecular profiles with Neighbor-Joining method. Results The genetic variability among trypanosome isolates examined was obvious with an average genetic distance of 41.2% (ranged from 0 to 100%). Similarity coefficient among T. brucei isolates was 41.15% which was lower than that between T. evansi and T. equiperdum isolates. The closest relationship was found between T. evansi and T. brucei isolates with a similarity coefficient of 62.94%. The genetic variability between T. b. rhodesiense and T. b. brucei isolates was higher than that among T. b. gambiense isolates. Conclusion Species and subspecies in Trypanozoon displayed a higher genetic variability; T. equiperdum isolates collected from China and from South America, and T. evansi isolates from China and from South America, should have a similar origin.
